Thank you for booking your New Patient Appointment, this is the first step to optimizing your health care and we are excited to be a part of that journey!
Please note forms are REQUIRED 48hrs prior to your new patient appointment.
Failure to complete new patient forms will result in the need to reschedule your appointment to a later date.
Upon completion of your forms you will receive a check mark informing you they have been received by the office. Please Note a Credit Card is required to hold and confirm the first appointment as noted in the financial agreement policy.
A fee in the amount of $140.00 will be applied to the provided card if you no show your appointment or cancel in under 48hrs notice.
If you are unable to complete forms online we will need for you to print your forms or to come in earlier for your appointment to complete them in office. Please call the office and request that forms be emailed to you. Print, Complete and Bring the forms to your appointment.
